Severe Mental Illness (SMI)

What is the Severe Mental Illness (SMI) Service?

Brunel Health Group (BHG) and Swindon and Gloucestershire Mind are working together to increase uptake of Annual Health Checks (AHCs) for individuals with Severe Mental Illness (SMI). These checks are vital to detect early signs of health conditions such as heart disease and diabetes – conditions that disproportionately affect people living with SMI.

Our goal is to reduce health inequalities and ensure that patients in our community receive the care and support they need.

How Can I Access the Severe Mental Illness (SMI) Service?

What can we offer?

Patients will receive:

  • A full physical health check: including blood pressure, weight, BMI, and blood tests
  • A discussion about smoking, alcohol, diet, and exercise
  • Support from a Wellbeing Practitioner to talk through mental health concerns, support needs, and referrals to other services if needed
  • This is not just a medical check-up – it’s a whole-person approach to wellbeing.
  • Pre-appointment contact from SG Mind to reduce anxiety and build trust
Who can access the service?

This service is available to all patients with SMI registered at BHG practices, with a particular focus on communities within the Core20PLUS groups:

  • Core20 Wards in Swindon: Penhill & Upper Stratton, Gorse Hill & Pinehurst, Walcot & Park North, Liden & Park South, Central, and others
  • PLUS Population: Adults from minority ethnic backgrounds
How can I make a referral?
  • You must be registered with the Brunel Health Group, and you can also contact them directly for information about your SMI Health Check.

Location & Opening Hours:

Our Saturday Clinics, will take place at Swindon Health Centre, from 9:00 AM – 1:00 PM.

Our extended offer will also include weekday clinics:

Our Weekday Clinics will take place at The Junction (S&G Mind) – typically Wednesday to Friday (for further information please email [email protected]).

These clinic settings offer both clinical and non-clinical environments to help patients feel comfortable and supported
