Severe Mental Illness (SMI)
What is the Severe Mental Illness (SMI) Service?
Brunel Health Group (BHG) and Swindon and Gloucestershire Mind are working together to increase uptake of Annual Health Checks (AHCs) for individuals with Severe Mental Illness (SMI). These checks are vital to detect early signs of health conditions such as heart disease and diabetes – conditions that disproportionately affect people living with SMI.
Our goal is to reduce health inequalities and ensure that patients in our community receive the care and support they need.
How Can I Access the Severe Mental Illness (SMI) Service?
What can we offer?
Patients will receive:
- A full physical health check: including blood pressure, weight, BMI, and blood tests
- A discussion about smoking, alcohol, diet, and exercise
- Support from a Wellbeing Practitioner to talk through mental health concerns, support needs, and referrals to other services if needed
- This is not just a medical check-up – it’s a whole-person approach to wellbeing.
- Pre-appointment contact from SG Mind to reduce anxiety and build trust
Who can access the service?
This service is available to all patients with SMI registered at BHG practices, with a particular focus on communities within the Core20PLUS groups:
