Swindon and Gloucestershire Mind are Seeking an Recovery and Wellbeing Coordinator
Job Purpose – The Recovery and Wellbeing Coordinator will work closely and in partnership with Avon & Wiltshire Partnership (AWP) NHS Trust, Swindon Recovery Team providing support from the Steps programme which involves outreach work and 121 wellbeing planning sessions.
The Step Programme involves:
- Step Down: Providing monthly wellbeing appointments following recent discharge from secondary mental health services for up to 12 months
- Step Up: Providing temporary added support at a time of crisis, usually for two weeks. The support is tailored to the individual and can involve appointments, 121 support and phone calls.
- Stepped Plan: Support is arranged for a set period which is no covered by the two other step approaches. It could involve outreach in the community, building structure to the week or engaging in community activities.
Hours: 28 hours (4 days per week)
Salary: £25,000 (FTE per annum)
Location: Swindon (SN1)
Reports to: Wellbeing Lead, Swindon
Benefits: 25 days annual leave plus 8 days bank holiday entitlement (pro rata for part time staff), pension scheme, ongoing training and development opportunities and team away days to support a culture of wellbeing and teamwork.
